STUDENT SPECIAL SCHOLARSHIP PROGRAM |

I. NAME, MISSION, & PURPOSE A. Name: The Philippine Nurses Association of Northeast Florida, Nursing Student Special Scholarship Program B. Mission: To inspire Filipino nursing students to complete a nursing degree. C. Purpose: This scholarship program is intended for currently enrolled nursing students. This scholarship is a one-time grant to help students who need assistance in purchasing books and related school expenses, excluding tuition. PNANEF will award $ 500 scholarship grant to qualified applicants. II. SCHOLARSHIP COMMITTEE A. The scholarship committee shall formulate the criteria for selection of candidate. B. The chairperson shall be appointed by the President of PNANEF. C. The committee shall monitor the academic record of scholarship recipients. D. The scholarship award shall be a one time grant for educational expenses, excluding tuition. E. The scholarship committee may propose changes
to current policy related to the scholarship program. III. CANDIDATE SELECTION PROCESS A. Selection Criteria 1. Evidence of eligibility. a. The applicant must be a Filipino by birth or affinity b. Must be a resident of Northeast Florida c. Must be an associate member of PNANEF and involved in its projects ( include active involvement in PNA activities and/or other community services). d. Must show proof of financial need. e. Must be currently enrolled in an accredited Nursing Program f. Must have done clinical work for at least one term or semester. g. Thorough completion of application form 2. Recommendation letters (2) a. Purpose clearly stated b. Express need for financial assistance c. Express good moral character and integrity d. Potential completion of program (based on academic aptitude). 3. Candidate plan and/or acceptance to an accredited school. a. Clear plan and/or projected dates of school attendance. b. Evidence of current enrollment in a nursing program (submit a letter from the school of nursing showing enrollment in a clinical course) B. Scoring Each applicant will be given two (2) points per item under each of the 3 general criteria. The minimum points to qualify for scholarship shall be 16 and the maximum points shall be 20. Each Committee member shall evaluate the application based on specific selection criteria. Applicants with an average score of 16 or less shall no longer be included in the selection process. Upon the discretion of the scholarship committee, an interview may be requested to clarify concerns and issues related to the application process. C. Selection of Recipients The name/s of successful applicant/s will be released within 30 days after the Committee chairperson performs a final review of the candidate selection process. The Committee chairperson shall submit the names of successful applicants to the Executive Board for approval. III. ANNOUNCEMENT, AWARD, AND DISBURSEMENT OF FUNDS A. Announcement and awarding of scholarship grant Upon approval of the scholarship committee endorsement by the Executive Board, the President shall send an official letter to qualified applicants regarding scholarship approval, scholarship availability and disbursement of scholarship funds. The President shall coordinate with the Treasurer in the release of the scholarship funds to qualified recipients following guidelines for disbursement of funds. B. Disbursement of funds Qualified nursing students who have met the above criteria and who successful passed the screening process shall be awarded $ 500 (five hunded dollars ). The Treasurer shall disburse funds directly to the student following disbursement protocol established by the Executive Board. Qualified nursing students who received this scholarship grant will be responsible to submit a statement of expenses showing proof of appropriate use of funds. The Treasurer may request a copy of this statement for PNANEF records. V. DISQUALIFICATION AND WITHDRAWAL A. In the event that the scholarship recipient fails to complete the program and/or decides that she/he no longer wishes to continue the scholarship program. The recipient shall be held responsible to notify the committee on scholarship in writing regarding intent to withdraw from the program. Failure to notify the scholarship committee shall subject the recipient a penalty such as the return of the scholarship money to PNANEF. B. Grounds for disqualification shall be established if and when the scholarship recipient willfully participates in fraudulent acts, or misrepresentation during the period of scholarship endowment. VI. PUBLICITY A. Announcement of recruitment and selection of candidates shall be made during the General Membership Meeting and then published on the PNANEF website:www.pnanef.com B. Timeline 1. Application forms shall be available all year round. 2. Application forms may be available through the PNANEF website. You may also request a mail-in application form from your PNANEF chairperson, committee members, or from PNANEF members with internet access. 3. Deadline for return of completed application forms to the Scholarship Committee: April 15 and September 15 4. Notification of Scholarship Recipient(s): Last week of June and November VI. LIABILITY The PNANEF shall be held harmless for any accident, illness or injury incurred while student is under the limited scholarship funding and a statement to this effect shall be signed by the recipient.
